MOC Eligibility Quiz
Are you currently employed as a GME professional of an ACGME, ACGME-I or ACGME NST program?
Have you completed five (5) consecutive years in this role by the date of application submission?
Have you obtained 25 qualifying Education Credits (EC) within the previous 5 years from the date of application submission?
Applications are submitted via TAGME’s application portal.
The application consists of several sections. It is important to complete all required fields. Incomplete applications will be denied. Due to the complexities of institutional email security measures, primary communication from TAGME will be to the candidate’s personal email account. Be sure to enter your name on the application form as it appears on your valid, government-issued picture ID.
Supportive Verification Form
All applications require a Supportive Verification Form (SVF) to be completed by the Program Director, GME Office, DIO, Department Chair, and/or Dean. Within the application, you will be able to choose to deliver the SVF in an online format or to print a PDF for upload upon completion.
Pay the Examination Fee
Complete the application process by paying the $350 exam fee.
Exam Scheduling
Upon approval of the application, supportive documentation, and successful payment processing, candidates can expect to receive an email on or about July 5, 2025, containing detailed information to schedule the initial exam.
Rescheduling or Cancelling the Exam Appointment
Candidates needing to reschedule their original assessment appointment may do so with no less than 24 hours without penalty. If you need to reschedule your assessment after the 24 hours allowed, please login using your Survey Monkey (https://tagme.smapply.org) User ID and password and then click on PROGRAMS in the upper right-hand corner.
Study Guide
Questions for all exams are sourced from the Study Guide. It is strongly advised that candidates study these documents in detail and become comfortable accessing the information quickly and accurately. Printed copies of these documents are admissible during the testing session. PDF versions of the Study Guide files will also be available onscreen within the secured exam browser.
Exam Day
Online, proctored Exam: At the scheduled assessment time, candidates will log into the testing system using the unique login and password provided in the email confirming the assessment appointment.
In-person Testing Center: Candidates choosing to use a Testing Center will receive an email with details provided by Meazure on where to report as well as instructions and expectations.
Upon successfully passing the Maintenance of Certification Exam, the certified professional will receive an email from Parchment with instructions to access their digital certificate and to verify their address for the hard copy to be mailed.

I was unaware of a limited application window for the MOC exam. My TAGME certification expires at the end of this year. Can I still apply? | No. Missing the deadline means your certification will expire on 12/31/current year. You must reapply for the Initial examination the following year. If you do not choose to apply, you will remain uncertified and unable to use the C-TAGME credentials. |
I missed the MOC exam application deadline. Can I appeal? | There are no exceptions or appeals given. There are two options: 1. You must wait for the next cycle and apply for the Initial Exam, starting over again. OR 2. Pay the MOC Late fee of $600.00 if within the late application window. When I completed the application and was approved, I did not list the accommodation at that time as they were not needed. A few months have gone by, and now I have a medical issue and would like to update my application. I now need accommodation. What should I do? | Submit a physician-signed letter to applications@tagme.org at least 30 days before the exam. Unexpected cases should be reported immediately. This will ensure time to contact the testing site and Proctor for notification and any requested setup. If this is an unforeseen accident/illness request, please inform the Applications Committee immediately. If the documentation is not sent promptly, you may be asked to reschedule your exam for a different date. |
I will need to take breaks to breastfeed or pump in a private area. How does this work for remote and/or testing centers? | You will need to request additional time for accommodation as you will need to leave the testing center exam room; extra time is allotted on top of the scheduled breaks for breastfeeding. For virtual testing, the room and security measures will be enforced each time you leave the room. Each testing site will have secure requirements to comply with to continue each time you reenter the testing site. The clock will continue to run with any additional time allotted. |
Can my dog be in the room while I take the exam? | The dog can only be in the room (home or testing site) if it is a certified SERVICE dog. We do not want anything to disrupt you during the exam. We must see a copy of the official Certified Service Dog card with the issuing agency’s phone number. |
